In-person / In-office appointments are scheduled on Fridays. Telemedicine appointments are available Monday through Thursday, and occasionally Saturday.

No insurance is accepted at this time.
Reimbursify.com is a useful app to submit out-of-network claims to your insurance company for reimbursement. Superbills are provided automatically at end of month.

The practice offers "second opinion" consultations regarding treatment resistant depression and OCD. Tools considered during the evaluation process include: pharmacogenetics and advanced neuromodulation modalities including rTMS (repetitive transcranial magnetic stimulation), vagal nerve stimulation, and deep brain stimulation.
Legal / forensic opinion requests should be initiated by legal counsel.
Not provided: Child and Adolescent psychiatry, management of acute suicidality or crisis services, management of psychotic disorders, cases requiring social work or case management services, severe and active eating disorders, step down from acute hospitalization, one time FMLA or disability evaluations, active withdrawal states or severe substance dependence, evaluations where there is an active custody case open, letters for emotional support animals, any case that Dr. Pisklak assesses as out of the scope of this private practice or more safely managed at higher level of care that can be provided through outpatient solo practice or telemedicine.
